ABIC Event - Converting and Combining Patient Records with Katie McCarthy

Managing the number of potential duplicate patient records within an electronic health record system is never an easy task. In fact, some estimate that 11-15% of patient records within a system are duplicates. And, if that number is close to correct, then how does this affect reporting and analytics? And how is this made more complicated when patient records are obtained from multiple data sources? During this presentation, we will answer these questions using an example of an enterprise project within a large health system where the goal was to combine three electronic health record systems into a single system. This conversion required multiple steps for determining scope, identifying patient records to be converted, and determining the number of potential duplicated records. Specifically, we will discuss several tools that supported this process including a confidence score matrix and a risk stratification for duplicate records and allowed the organization to convert more than 7.6 million records with an average error rate of 0.5%. In addition, we will share lessons learned related to the impacts on reporting and analytics.
